Blepharoplasty
A small operation with a disproportionate effect, because the eyes are what people look at first. Done well, you look rested rather than operated on.

01 · Understanding the concern
Why patients consider this
Upper lid hooding makes people look tired regardless of how they feel, and at a certain point it starts to interfere with the upper field of vision or with applying makeup. Lower lid changes are different: the complaint is usually shadowing and puffiness that photographs badly and does not change with sleep.
02 · The procedure
What blepharoplasty involves
Upper blepharoplasty removes excess skin, and sometimes a small amount of muscle or fat, through an incision hidden in the natural lid crease. Lower blepharoplasty addresses fat that has prolapsed forward, either through an incision just below the lash line or from inside the lid, with fat repositioned rather than simply removed where the tear trough is hollow. The approach depends entirely on which anatomical problem is present.
03 · Candidacy
Who may be considered
This is general guidance rather than a diagnosis. Suitability is decided by examination.
- Excess upper lid skin causing hooding or affecting vision
- Under-eye fullness that is anatomical rather than fluid-related
- Good eye health, with dry eye assessed beforehand
- Realistic expectations, since eyelid surgery does not treat crow's feet or dark pigmentation
- Medically fit for the anaesthetic planned
What it can address
Concerns it is used for
- Hooded upper eyelids
- A tired appearance despite adequate rest
- Under-eye bags and shadowing
- Loss of the upper lid crease
- Upper visual field obstruction in more advanced cases

04 · Planning
Why the planning matters more than the technique
Brow position is assessed before the lids. Removing upper lid skin when the true problem is a descended brow pulls the brow lower still and makes the appearance worse. Similarly, dark circles caused by pigmentation will not change with surgery, and saying so beforehand prevents disappointment afterwards.
05 · How it works
Step by step
- 01
Assessment
Lid, brow and tear trough assessed together, with dry eye and lid laxity checked.
- 02
Marking
Skin excess measured and marked with you sitting upright.
- 03
Upper lid
Excess skin and, where needed, fat addressed through the lid crease.
- 04
Lower lid
Fat repositioned or reduced, via skin or transconjunctival approach.
- 05
Closure
Fine sutures placed in the crease or lash line.
- 06
Review
Suture removal and scheduled follow-up.
06 · The procedure journey
Before, during and after
Before
Eye assessment where indicated, investigations, and stopping medication that increases bruising as advised.
During
Often local anaesthetic with sedation for upper lids; general anaesthetic for combined or more extensive work.
After
Swelling and bruising around the eyes for one to two weeks, with tightness and dryness common early.
07 · Recovery
What recovery generally involves
Sutures are typically removed within the first week. Most people are socially presentable at around ten days to two weeks, though bruising varies considerably. Vision may be blurry initially from ointment and swelling. Fine scars in the lid crease settle well in most patients.
Recovery times are general ranges. Your own timeline depends on the extent of treatment, your health and your work, and is given at consultation.
- Cold compresses in the first 48 hours as instructed
- Sleep elevated for the first week
- Lubricating drops as prescribed
- No eye makeup until cleared
- Sunglasses outdoors while healing
08 · Outcomes
What to expect from the result
A more open, rested eyelid appearance. Surgery does not treat fine lines, pigmentation or brow position, and ageing continues. Results vary and are not guaranteed.
09 · Safety
Risks and considerations
Risks include dry eye or worsening of existing dry eye, asymmetry, over- or under-correction, lower lid malposition, temporary blurred vision, and very rarely serious visual complications. Pre-existing dry eye and lid laxity are assessed specifically because they change the risk.
No procedure is risk-free. A full, individual discussion of risk forms part of consent and takes place before any decision is made.

Questions patients ask
Blepharoplasty: common questions
Will eyelid surgery remove my dark circles?
Not if they are caused by pigmentation. Where shadowing is caused by fat prolapse and hollowing, surgery can help.
Do I need a brow lift instead?
Sometimes. If the brow has descended, treating the lid alone can worsen the appearance. Assessment covers both.
How long is the bruising?
Commonly one to two weeks, though it varies. Most patients plan around ten days before significant social commitments.
Is upper eyelid surgery done under general anaesthetic?
Often it is performed under local with sedation. The choice depends on the extent of surgery and your preference.
Will the hooding come back?
Ageing continues, and some recurrence over years is possible, though repeat surgery is uncommon for most patients.
Can it be combined with other facial procedures?
Frequently, yes. Combination is planned according to total operating time and recovery.
