Gynecomastia Surgery

Male chest fullness that does not respond to training is usually glandular, not fat. That distinction decides whether surgery is the answer and what kind of surgery it is.

01 · Understanding the concern

Why patients consider this

Gynecomastia is common and frequently under-discussed. Many men train for years expecting the chest to flatten and find that it does not, because firm glandular tissue behind the nipple does not respond to exercise or weight loss. It affects clothing choices, swimming, and in many cases confidence, long before anyone asks for help with it.

02 · The procedure

What gynecomastia surgery involves

Surgery removes the glandular component, usually through a small incision at the edge of the areola, and addresses surrounding fat with liposuction so the chest contour blends into the chest wall. In higher grades with significant skin excess, skin reduction may also be required, which changes the scar pattern. The two components are almost always treated together, because removing gland alone leaves a hollow and removing fat alone leaves the firm disc behind.

03 · Candidacy

Who may be considered

This is general guidance rather than a diagnosis. Suitability is decided by examination.

  • Firm tissue behind the nipple that has not changed with training or weight loss
  • Stable condition for a reasonable period
  • No untreated underlying cause identified on assessment
  • Non-smokers or those able to stop as advised
  • Medically fit for anaesthesia

What it can address

Concerns it is used for

  • Firm glandular tissue behind the areola
  • Puffy or protruding nipple appearance
  • Chest fullness that persists despite low body fat
  • Asymmetry between the two sides
  • Skin excess in higher grades

04 · Planning

Why the planning matters more than the technique

Grade determines technique. Lower grades are managed with gland excision and liposuction through small incisions. Higher grades with skin excess may need skin reduction, and the scar trade-off has to be discussed and agreed before surgery rather than discovered afterwards.

05 · How it works

Step by step

  1. 01

    Assessment

Examination to determine the proportion of gland to fat, the grade, and any cause needing investigation.

  2. 02

    Marking

Contour and areolar position marked with you standing.

  3. 03

    Liposuction

Surrounding fat is treated first to define the contour.

  4. 04

    Gland excision

Glandular tissue is removed through a periareolar incision, leaving a thin layer to avoid a hollow.

  5. 05

    Skin management

Where skin excess is significant, a reduction pattern is used as agreed.

  6. 06

    Compression

A compression vest is fitted before you leave.

06 · The procedure journey

Before, during and after

Before

Investigations, and where indicated, blood tests to look for an underlying cause. Medication history is reviewed, since some drugs contribute.

During

Usually general anaesthetic, commonly as a day case depending on grade and extent.

After

A compression vest, bruising and swelling across the chest, and soreness with arm movement for several days.

07 · Recovery

What recovery generally involves

Most patients return to desk work within about a week. Upper-body training is restricted for several weeks. The compression vest is worn as instructed, usually for some weeks, and matters for contour. Swelling resolves over months and the final chest shape is assessed later rather than sooner.

Recovery times are general ranges. Your own timeline depends on the extent of treatment, your health and your work, and is given at consultation.

  • Wear the compression vest for the full period advised
  • No chest or shoulder training until cleared
  • Expect temporary nipple numbness or hypersensitivity
  • Scars mature over many months
  • Avoid weight fluctuation while healing

08 · Outcomes

What to expect from the result

The aim is a flatter, firmer chest contour and a nipple that sits against the chest wall. Glandular tissue removed does not return, but weight gain can change the surrounding contour. Results vary and are not guaranteed.

09 · Safety

Risks and considerations

Risks include haematoma, infection, contour irregularity or over-resection leaving a saucer deformity, asymmetry, changes in nipple sensation, and scarring. Higher-grade cases carry additional considerations around skin reduction and scar position. All of this is covered at consultation.

No procedure is risk-free. A full, individual discussion of risk forms part of consent and takes place before any decision is made.

11Questions

Questions patients ask

Gynecomastia Surgery: common questions

How do I know whether it is gland or fat?

Glandular tissue feels firm and disc-like behind the nipple and does not change with training. Examination at consultation makes the distinction.

Can gynecomastia be treated without surgery?

Where a medication or underlying condition is responsible and the condition is recent, addressing the cause can help. Established glandular tissue generally does not resolve on its own.

Will it come back?

Removed glandular tissue does not regrow. Significant weight gain or a new underlying cause can change the appearance.

What will the scar look like?

In most cases a short scar at the lower edge of the areola, which usually settles well. Higher grades requiring skin reduction have longer scars, discussed beforehand.

When can I return to the gym?

Lower body work is usually resumed earlier than chest and shoulder work. Specific timing is given in your aftercare.

Is the surgery covered by insurance?

Coverage depends on your policy and whether the condition is documented as symptomatic. The clinic can provide documentation, but the decision rests with the insurer.
